If this is your first time logging in, your username is your
license number, and your password is your last name.
For example John Smith, Jr. with license number 1234 would use this:
license number: 01234
password: smith
Note: If you have a four digit license number put a leading zero in
front of your license number to log in. If you have a five
digit license number just put in the five digit number without
leading zeros.
After you
login for the first time, you will be asked to change your password.
